Toprol 50 mg vs. 100 mg does it matter? I used to take Toprol 50 mg/day which lowered my BP to average 136/85 (from a baseline of 144/92). My doc increased my dosage to 100 mg/day but my BP has not lowered a bit, still averaging 136/85. Why no improvement?

greengo replied: "Assuming you have had time to reach a steady state with the new dosage, it may be that you won't get much more lowering with any higher dose. Drug efficacy tends to plateau out at a certain point, and then increased dose doesn't really help the problem but may only increase the side effects. You should be on the lowest effective dose. Usually toprol is titrated to pulse. If your pulse is lowered to around 60, then you are maxed on that drug. I think what I would do if I were your doctor is to decrease toprol down to 50 and add another drug such as a thiazide diuretic. Or possibly try another type of drug altogether."

gangadharan_nair replied: "Metoprolol is used alone or in combination with other medications to treat high blood pressure. It also is used to prevent angina (chest pain) and to treat heart attacks. Extended-release (long-acting) metoprolol also is used in combination with other medications to treat heart failure. Metoprolol is in a class of medications called beta blockers. It works by slowing the heart rate and relaxing the blood vessels so the heart does not have to pump as hard. The doctor might have considered the optimum blood pressure in your case as 130/80 or 120/80 mm Hg. Hence he has increased the dosage. Consult your doctor. I have been prescribed Toprol XL 50 mg, Lexapro 10mg, and Ativan 0.5 mg for my Anxiety, High Blood Pressure? and depression. What kind of differences will these medications have on me compared to when I was taking Atenlol 25mg and Prozac 100mg. The later combination was only increasing my depression which is why my doctor changed my meds. Thnx.

Barberry replied: "I don't understand why your physician put you on Ativan AND Lexapro. Ativan should be used for acute episodes, short term. Are you seeing a psychiatrist or just your family doctor? Sometimes family docs aren't up on the latest treatments for mental health issues. No one can really tell you what to expect from your medication combinations. Everyone reacts differently to meds, so you'll need to see how you're feeling after a few weeks on that regimen. I have known some people to do poorly on Prozac but tolerate Lexapro much better. Some people don't do well with Ativan, others do. Atenolol can cause depression in some people. Toprol is a more "modern" drug and has shown good results. From what I've read, there is less negative reaction with Toprol and Lexapro than there is wtih Toprol and Prozac. Hopefully your doctor will be monitoring you closely for any side effects and to make sure the Toprol is doing the job for your HTN. Hopefully he/she has also given you diet counseling as far as helping control your high BP. Exercize is also imperative for you, to help reduce your BP and to help relieve symptoms of anxiety and depression. Even walking 20 minutes a day can make a difference. Good luck."

Pico replied: "I don't know anything about Toprol other than I believe it's for things like hypertension. I was started off on 10mg of Lexapro for depression and 0.5mg of Ativan for my panic attacks/anxiety. I'll be honest and say I got addicted to the Ativan very quickly. The Lexapro did nothing for me until I was raised up to 20mg. I'm on 30mg now. There's nothing wrong with being on Lexapro and Ativan at the same time. The Lexapro did nothing for my panic attacks and the Ativan was a 'take as needed' pill. If I felt a panic attack or extra anxiety coming on, I took a pill. I was not to exceed 2 pills a day but I started popping 6 or 7 (the addiction...) I now take Klonopin instead for the anxiety because it's longer lasting and less addictive for me. As for comparing these meds to the ones you were on before, that's hard to answer. All meds work differently for everyone. I had been on 200mg of Zoloft and it did NOTHING for me. The 30mg of Lexapro I take works 100 times better than the Zoloft ever did."

difference between inderal 40mg and toprol xl 50? I am wondering what is the dosage difference between Inderal 40 mg and Toprol XL-50 mg. Which is the better one to take long term?

hotmama replied: "Inderal LA? They are both long acting...if your talking about inderal la, the capsules,there is plain inderal tablets too(generic is propranolol..i think??). Inderal LA and Toprol XL are both long acting, LA (long acting), XL (extended release) except toprol are tablets. They work in the same way,just different medications. Inderal is made in 20mg doses, 20mg,40mg,maybe 60mg,and 80mg...and so on. Toprol is made in 25mg,50mg,100mg and 200mg. Even though a medication works in the same way as another, it may be required to take a higher dose of one to make it work as well as the other did."

Toprol and high blood pressure? I have been taking toprol 50 mg for about a year. I few times I have forgotten to take my pill & by 10:00 am I feel like my heart is racing and like I am having a hard time breathing. About 20 minutes after I take my pill - I start feeling better. Is my body getting used to this medication - how did I live without it. Does anyone out there take Toprol? I am 56 years old, had blood pressure 150/90 before taking toprol and a very rapid heart beat - between 100 & 120 beats a minute. I have had a rapid heart beat since my 20's. Had a stress test done and it showed my heart was ok... Thanks :)

Stephanie F replied: "My Dad takes Toprol since his heart attack and his open heart surgery. They told him that he will always have to take it. You can ask your doctor and discuss with him/her your concerns. My Dad said if he doesn't take it, his heart races also. I don't know about his breathing. He was just diagnosed with emphazema. My Dad is 66 years old and his surgery was 3 years ago. Best wishes to you and remember, everyones body reacts differently to different medicine. I had a triple bypass surgery at the tender age of 39. With diabetes and now my kidneys have quit and dialize three days aweek.But i still don't know why our head feels the way it does sometime."

mutantalbino replied: "Hello, I take 25MG daily to regulate my heart rate. I also take Altace for the same thing. While it lowers blood pressure, that is not the primary reason I take it. My resting pulse was around 110 after CHF. I never noticed a particular problem if I took it an hour or two late. Of course I take Altace in the PM which may be a factor."

Hubby is on Vancomycin- can dosing too fast cause renal failure? Hes taking it for a staff infection in his foot. He also has rheumatoid arthritis and was on Enbrel prior to infection and antibiotic. Hes got high blood pressure. He was taking Toprol 50 mg a day and Avalide 150 mg a day for that. They changed his IV at home from 1 gram twice a day to 2 grams once a day. No one ever told him to infuse slower! So in the first 15 min he got extremely itchy and rash all over - redmans syndrome. About 8 hours later we took him to the ER because he had a fever of 102. Turns out he was in full renal failure. I read that blood pressure med plus vanco and serious injury or illness can cause renal faliure. What I want to know is because he infused to fast err on part of the Home Health agency, did that cause him to go into renal failure?

Fisher replied: "Vancomycin can lead to renal failure if the levels get too high. His renal function should have been checked before starting vancomycin treatment. I assume the doses were adjusted to account for a change in his renal function. Infusing the vancomycin too fast would not lead to renal failure itself. Vancomycin could have contributed to the renal failure if his kidney function worsened for some reason causing the vancomycin level in his blood to elevate to high- (this would happen because his kidneys were not removing it from his blood as quickly as before, not from infusing it too fast. It would take a few doses to get it that high.) Then the extra high vancomycin blood levels could contribute. There are some blood pressure medications that can cause decreased renal function. The ACE inhibitors like lisinopril, enalapril, etc can decrease kidney function which could lead to higher vancomycin levels, which could further contribut to the renal failure. This type of renal failure is usually reversible after the medications causing the failure are removed. I hope that made sense- It sounded redundant to me but is is difficult to explain simply in a couple of paragraphs."

I suffer from a lot of espisodes of tachycardia during my pregnancy.. how often should I get an echo? I read something about peripartum cardiomyapathy.. and can be developed in the 3rd trimester.. I am currently 25 weeks pregnant.. on a beta blocker (toprol 50 mg) and my last echo was normal 1 month ago.. should I still worry?

cari_co26 replied: "I would say that you are probably fine. I didn't do a ton of research on peripartum cardiomyapathy, but I did not see where tachycardia can be a cause. What kind of tachycardia do you have? Is it the more common sinus tachycardia? I know there are many kinds and severities but if your last echo was good then try not to worry. peripartum cardiomyapathy is very rare... and you are doing good. Stress won't help you though. Try to relax and kick back and enjoy being pregnant. Think about light things, names for the baby, color of nursery, what he/she will be like. Just do what you can, eat right, relax and if you have a heart condition, then take it easy. Just enjoy and remember to change what you can and accept what you can't. But personally, based on what I know, I think you are fine now, and I think you will continue to be so. Don't borrow trouble. ;)"

ralphrepo_01 replied: "PeriPartum Cardiomyopathy (PPCM) is an extremely rare form of heart failure that appears for no other discernible reason except that it occurs during the last trimester or even shortly after pregnancy. It is suggested that the cause may be a form of myocarditis, but no one really knows for sure what the etiology is. Treatment for it remains consistent with the goals surrounding treatment of general heart failure, but with the added complexity of maintaining an environ suitable for continuation of the pregnancy. The incidence in the US is estimated as one in 15,000 births (by comparison, Down's Syndrome is about 1:1000 chance). That said, having a few episodes of a fast heart beat does not make a diagnosis of PPCM. If performed, a chest X Ray (with fetal shielding) usually reveals an enlarged cardiac outline, with evidence of fluid congestion. Echocardiograms reveals a loss of left ventricular contractility, with a drop in ejection fraction; and EKG may show (in addition to tachycardia) non specific ST-T wave changes, a widened QRS complex, and or various forms of heart block. In addition to beta blockers, treatment can include diuretics, cardiotonics (digoxin), vasodilators, and others depending on the severity of the symptoms and the physician's knowledge of the pt's progress. Again, a few episodes of tachycardia doesn't mean anything. You're being followed by a doctor so you're already doing the right thing to ensure that the pregnancy will go well. Ct scan hospital ok. I'm thinking a cervical disc. problem for the headache. Transported because advice from medics and store

As you can see, being transported to the hospital, a bad headache, and high blood pressure is not on the list. This almost certainly was not an allergic reaction. Classic allergic reactions are those that cause you to have hives and low blood pressure. Why exactly did you go to the hospital? What happened once you were at the hospital...they treated your headache or blood pressure or both? Your blood pressure was elevated but it was not critical - and usually would not be responsible for the headache at that level (but might be the other way around - the headache caused the high blood pressure). Do you have a history of headache? Did your doctor perhaps take you off of one blood pressure medication when he/she started you on Toprol? Do you have a history of high blood pressure? While PVC's can be improved with b-blockers if a patient finds them disturbing or if there are "runs" of PVCs occurring in bunches. Short of those two reasons, PVCs generally do not require treatment. Your question seems to generate more questions than it answers. This was not an allergic response to Toprol, though. Good luck. Addendum: In light of the adrenal adenoma, I concur with the trip to the hospital. Usually these are classified in terms of whether they are hormonally active and whether they are malignant. I suspect, therein lies the answer to your symptoms...(glad to hear the head CT was negative) please discuss this further with the physician who is managing your condition. The way you describe your headache, by the way, is a classic de ion of a "stress" headache. Cervical spine dysfunction certainly can be a trigger and/or exacerbate a stress headache. My best. Should I take Toprol XL? My doc has given me Toprol XL 50 mg to take prior to having a chest CT - I am suppose to take one the night before and one the morning of the CT. I do not have HBP, actually have low BP. How will this affect my heart rate??

quijibored replied: "You don't say what the CT scan is for so we can only speculate whether or not the Dr had a reason to give you a beta blocker other than to relax you a little bit. Did he offer to give you Valium and you refused to take one? If so, he might have prescribed the Toprol as an alternative. Your BP will probably be monitored during the CT scan and the technician will be watching you closely to make sure everything is ok. Whatever the situation, two doses of Toprol XL is no big deal, so follow the Dr's orders. CT scans are not a big hassle anyways. MRIs are 100 times worse."

Drews replied: "Toprol XL is a selective beta blocker (meaning it blocks epinephrine and norepinephrine from binding to the Beta 1 receptors). Since Epinephrine is the chemical released during fight or flight responses, it is responsible for increasing your heart rate. For example, someone jumps out and scares you, your heart will begin to race. This is the epinephrine at work. Toprol XL blocks the ability for epinephrine to cause your heart to race. This helps people who have heart conditions ranging from heart palpitations, heart failure, arrythmia, or just need to relax a little. As far as high blood pressure goes, this is not a commonly prescribed drug for high blood pressure. Toprol XL would be probably the 3rd type of drug prescribed for that. Usually, ACE inhibitors are first line, then ARB's (angio-receptor blockers), then beta-blockers. Toprol XL is a terrific drug and is the most commonly prescribed drug by Cardiologists. The XL meaning "extended release". That is how it maintains beta-1 selectivity without spilling over into beta-2. Other generic beta blockers like Atenolol are immediate release capsules and cause beta 2 spill over. Your heart rate will be normal. Hope this helps!"

chrs_grdnr replied: "toprol xl is for high blood pressure or angina. it's list of side effects includes low blood pressure, not high. so you should look at the other meds you're taking............maybe it's a bad combination with something else you're on. allergic reactions are usually, upset stomach, a rash, wheezing, hives etc. did you read the med info list that came (should have come) with the pre ion? that may help............"

Does Lopressor have any negative side effects? My husband switched from taking one 50 mg Toprol XL a day to taking 2 - 25 mg of Lopressor a day, about 2 weeks ago. He has been very tired lately, and has no energy. Could the Lopressor be the culprit?

mamason replied: "Could be it's knocking his blood pressure down too low. Call the doc. Could also be an allergic reaction. My mom had that to a blood pressure pill. Caused her heart to beat too slow. Not enough oxygen going throughout the body. They took her off the pill, tried a different kind, and she's just fine."

Stephanie replied: "Yes it could be. I've had the same problem. Normaly though the effects will level off in about 2 weeks or so and become less bothersome. If he is still having problems, he should talk with his doctor to see if there are other options for him."
